Through our many years of helping families we have collected a large network of experts. We work with the very best people we can find to provide you the high quality care and support you need and deserve.

Adult Residential Care

  • Registered nurse and social work visit monthly
  • Monthly physical assessment of patient by an R.N.
  • Medication monitoring
  • Assessment and reassessment of medical and psychosocial needs
  • Liaison with physician and regular communication with family/representative
  • Initial and updated multidisciplinary care plan
  • Assessing need for and obtaining durable medical equipment
  • Ongoing evaluation of caregiver's competency in nursing skills and ability to provide adequate daily care
  • Identifying and planning for long term care needs
  • Counseling and support to patients, caregivers and families
  • Monthly case management reports sent to family or representative

While not all of theses services are available to every participant, we promise to help access those which are medically necessary.
